Chad Edward Lewis of Lebanon, formerly of Dublin, departed this life Wednesday, Aug. 22, 2012, in Johnson City, Tenn., after a short illness.
Chad is survived by his parents, Charles “Ed” and Frances Lewis; his wife, Ashley Lewis; one brother, Carl Lewis; one sister, Carla Lewis Buckner; two daughters, Jocelyn Lewis and Taylor Lewis; one son, Dylan Jessee; one grandson, Peyton; mother and father-in-law, Scott and Susan Bostic, and numerous aunts, uncles, great- aunts, great-uncles, cousins and many friends.
Funeral services will be held Saturday, Sept. 1, at Dublin United Methodist Church in Dublin.

I am so sorry about Chad. I worked with him at Wal-mart. He was a good friend to me.The family will be in my thoughts and prayers.
